Health Homes has helped clients transition from living in cars to obtaining affordable housing. Others struggling with severe diabetes have received guidance in understanding their diagnosis, obtaining medication, and using it correctly. By supporting self-care, Health Homes helps clients reduce emergency room visits and hospitalizations.
